H1N1 Pre-Registration Process
· Go to H1N1vaccine@odh.ohio.gov
· To Pre-register go to the “Individual Login” box on the right and click the “New Individual/Family to Pre-Register” at the bottom of the box.
· A prompt will occur to enter an email address and create a username/password.
· Next will be patient information: name/address/phone/DOB, etc. and then a series of questions. Once “SAVED” is selected the registration has occurred. PRINT THE FORM.
THE FORM MUST BE BROUGHT TO THE OFFICE APPOINTMENT FOR THE H1N1 VACCINE.
